Product · Deficiencies
Open means someone still has it.
A deficiency starts from a record: a reading outside the limit, or a check that failed. It does not start from a mood. It ends when a named person closes it, with a note.

Held is not the same as forgotten
Some items should stay open. A hinge pin that can wait for the next dry week is not a failure of the software. It is a failure if the list becomes wallpaper. A held item needs an owner, a reason, and a date to look again. Supervisors set that date. Inspectors do not close someone else’s item. That split has been in place since 2.2.
Food plants draw a harder line
At Larkspur Foods, a food-contact deficiency blocks startup. A held item that is not on a food-contact surface can cross a shift. The record is what the supervisor reads at 4:40 a.m., not a phone call to the person who already went home.
Where it lives
The deficiency stays attached to the asset and to the record that opened it. Photos from the close sit on the same item. The behavior is written up in closeout.
